As a Home Health RN, you will manage an assigned caseload and provide personalized care that helps patients remain safe, independent, and comfortable at home. Work autonomy and flexible scheduling Meaningful one-on-one patient care Supportive and collaborative clinical team Comprehensive onboarding and mentorship Tuition reimbursement for eligible full-time employees Continuing education opportunities Medical, dental, and vision insurance 401(k) with employer match Paid time off and paid holidays Family and pet bereavement Pet insurance Opportunities for professional growth What You’ll Do Manage an assigned caseload of home health patients Complete initial evaluations and ongoing nursing assessments Develop, coordinate, implement, and update individualized Plans of Care Monitor patients’ physical, psychosocial, environmental, and safety needs Provide skilled nursing treatments and patient education Educate caregivers and family members on the patient’s care needs Communicate changes in condition to physicians, clinical leadership, and other care team members Coordinate care with nurses, therapists, social workers, physicians, and community providers Complete accurate and timely clinical documentation Maintain patient privacy and confidentiality Support safe transitions and continuity of care What You’ll Bring Current and unrestricted Registered Nurse license in Maine At least one year of clinical nursing experience Strong assessment, communication, organization, and documentation skills Ability to manage a caseload and work independently in patients’ homes Willingness to travel throughout Wells, York, and the assigned coverage area Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations. Must be able and willing to travel 50% Associate Degree or Bachelor’s Degree in Nursing preferred Previous home health or hospice experience preferred HomeCare HomeBase experience preferred Bring Your Expertise Home Bring your nursing experience directly to patients and families throughout the Wells and York area. The base salary for this position is $90,000 to $100,000 annually, based on the company’s good faith estimate at the time of posting. Actual pay will be determined based on factors such as education, experience, skills, and internal equity.
