At Mass General Brigham Home Care, you’ll step outside clinic walls and into meaningful, one-on-one care—helping patients regain mobility, independence, and confidence where it matters most: at home. As a Physical Therapist, you’ll have the independence to use your clinical judgment, manage your schedule, and build meaningful relationships with patients, without giving up the support of a strong team. Join our Paul Revere team Most of your visits will be in Dorchester, with additional coverage in Jamaica Plain, Mission Hill/Roxbury. You’ll work with a diverse urban patient population, helping people meet their goals in the environment where they live every day. Why you’ll like this team You have independence, but you’re never on your own. •A schedule that works for you: Full-time, Monday–Friday with no evenings, 5–6 visits per day, and the autonomy to manage your own schedule and appointments. •Convenient territory: In some areas, you can walk between visits, and city parking passes are provided for new team members. •Dedicated Admission PT support: Dedicated Admission Physical Therapists support new patients coming into the service line and help establish their plan of care, allowing you to focus more of your time on ongoing patient visits. •Ancillary support services: Scheduling coordinators, liaisons, and other support services help with the administrative and coordination needs of care, allowing you to spend more time focused on your patients. •Strong team culture: Work alongside a collaborative team that values communication, teamwork, and supporting one another. Weekly interdisciplinary pod meetings, monthly team meetings, and quarterly in-person team days help keep the team connected. •Support to grow: Ongoing training, education, and opportunities to collaborate across disciplines. Our onboarding is designed to help you feel prepared and confident working independently in the community. Your time matters, too Your schedule includes dedicated time for documentation, chart review, visit preparation, and care coordination. Our MGB electronic documentation system gives you easy access to doctors’ notes and orders, helping make communication and care coordination more seamless. Your safety matters Because our clinicians work independently in the community, we provide safety resources to support you in the field, including: •GPS-enabled safety tools •Real-time alert features •Security-supported key fob access What you’ll do As a Physical Therapist, you’ll provide one-on-one care in patients’ homes, helping them improve mobility, safety, and independence. You’ll: •Evaluate patients and develop individualized care plans •Provide PT focused on mobility, strength, balance, and function •Recommend fall prevention and home safety strategies •Educate patients and caregivers •Track progress and adjust care as needed •Collaborate with the interdisciplinary care team Qualifications What you’ll bring At least one year of experience in acute care, rehab, or home care as a Physical Therapist Current registration with the Massachusetts Board of Registration Strong communication skills—verbal, written, and electronic Comfort with technology and the ability to learn new systems The ability to prioritize, stay organized, and adapt to changing situations Willingness to travel throughout Mass General Brigham Healthcare at Home’s geographic area Additional Job Details (if applicable) Join a team backed by Mass General Brigham With the resources of the Mass General Brigham system behind you, you’ll have the support, tools, and opportunities to grow your career while providing high-quality care in patients’ homes.
