Competitive pay:   $65.00-$70.00/hour DOE Sign-On Bonus Available Relocation Assistance Available Full-Time and Part-Time opportunities available Flexible scheduling options Medical, Dental, Vision, and Life Insurance for eligible employees 401(k) with employer match Paid Time Off and Holiday Pay Continuing education and professional development opportunities Supportive and collaborative interdisciplinary team Strong clinical support and leadership Career growth opportunities Modern therapy resources and equipment Beautiful Lake County location with a strong sense of community Speech-Language Pathologist (SLP) Responsibilities Evaluate and treat residents with speech, language, voice, cognitive-communication, and swallowing disorders Develop and implement individualized treatment plans based on resident goals and clinical needs Conduct dysphagia assessments and provide evidence-based swallowing interventions Treat residents with neurological, cognitive, and medically complex conditions Collaborate with physicians, nursing staff, rehabilitation professionals, residents, and families Educate residents, caregivers, and staff regarding treatment recommendations and compensatory strategies Participate in interdisciplinary care planning and discharge planning activities Maintain accurate, timely documentation in accordance with facility, state, federal, Medicare, and reimbursement requirements Monitor patient progress and adjust treatment plans to maximize outcomes Support resident safety, communication, nutrition, and overall quality of life Qualifications Master's Degree in Speech-Language Pathology required Current California Speech-Language Pathologist License required Certificate of Clinical Competence (CCC-SLP) preferred or eligibility to obtain Strong clinical, communication, and interpersonal skills Compassionate, patient-centered approach to care Ability to work effectively within an interdisciplinary healthcare team Experience in Skilled Nursing Facility (SNF), Post-Acute Care, Rehabilitation, Long-Term Care, or Acute Care settings preferred Knowledge of dysphagia management, modified diets, and swallowing disorders preferred Experience with cognitive, neurological, and medically complex patient populations preferred CF candidates are encouraged to apply, depending on experience and state requirements About Lakeport Post Acute At Lakeport Post Acute, we are committed to helping residents achieve their highest level of function, independence, and quality of life.
