Overview The Social Worker strives to promote patient and family wellness, improved care outcomes, access to appropriate hospital and community resources, and manages, supports and develops comprehensive transitional care plans for patients with complex and psychosocial needs in Inpatient and Emergency Department (ED) environments. Responsibilities The Social Worker strives to promote patient and family wellness, improved care outcomes, access to appropriate hospital and community resources, and manages, supports and develops comprehensive transitional care plans for patients with complex and psychosocial needs in Inpatient and Emergency Department (ED) environments. Qualifications Schedule We are looking for someone who can commit at least two days a week (during business hours) and possibly a third from time to time.  Education Master’s Degree from an accredited social work program Required Work Experience No experience required Required 2 years of experience in an acute or post-acute setting Preferred Licenses and Certifications LMSW - Licensed Medical Social Worker - State Licensure in the state of Georgia Preferred Business Unit : Company Name Piedmont Augusta Hospital
