Company Overview The Staten Island Workforce1 Career Center is partnering with The Community Agency for Senior Citizens (CASC) to find qualified candidates for a Senior Care Manager in Staten Island, New York. The Community Agency for Senior Citizens (CASC) assists Staten Islanders age 60 and older in living as independently as possible within the community. CASC works closely with older adults while respecting their right to make decisions, honoring their life experiences and heritage, and recognizing the important role of family, friends, and community support. Position Overview The Senior Care Manager provides information, assistance, and support to family caregivers of older adults. This role is responsible for assessing caregiver needs, developing individualized caregiver support plans, coordinating services, and connecting caregivers with community resources. The Senior Care Manager works with caregivers and clients with a range of needs, including complex situations that require ongoing coordination and advocacy. This position plays an important role in helping caregivers access services and support that promote the well-being of older adults in the community. This position is located in Staten Island, New York. Key Responsibilities Conduct caregiver intakes, assessments, and reassessments Complete case closures as appropriate Develop and implement individualized caregiver support plans Coordinate services to meet caregiver and client needs Manage a caseload of complex, high-need clients Connect caregivers to community resources and provide referrals Offer guidance, advocacy, and support to caregivers seeking services Maintain accurate and timely case documentation in compliance with NYC Aging standards Requirements Master's Degree in Social Work or a related Master's-level degree Minimum one year of experience in a social service organization Experience working with caregivers and/or older adults (preferred) Fluency in a second language (preferred) Excellent written and verbal communication skills Detail-oriented with strong organizational skills Sensitive to the needs of older adults and caregivers Ability to work collaboratively while respecting diverse ethnic, cultural, and linguistic backgrounds Pay Range USD $29.80 - USD $31.25 /Hr.
