Registered Nurse (RN) - Program Coordinator, Heart Failure, FT, Day

Inspire health. Serve with compassion. Be the difference. Job Summary Coordinates a interdisciplinary plan of care or program to meet the needs of a specific patient population. Functions as a clinical, patient, and community resource, educator, and consultant. Participates in patient care activities for the patient population(s) served including direct patient care, indirect patient care, patient rounds, consultation, multidisciplinary care planning, and evaluation.   Collaborates with other health team members to develop, implement, and evaluate quality programs to meet the needs of population(s) served in a cost-effective manner. Develops, provides, and evaluates educational programs to meet the identified needs of the population(s) served including patients, families, staff, and the community. Monitors and ensures accuracy of documentation on patient records.  Maintains all required reports, records, and statistics related to program. Monitors the quality of care delivered to the population(s) served on a continual basis; identifies quality issues and integrates process improvement (P.I.) into practice. May p articipate in research/investigative studies when indicated. The previous accountabilities are not an exhaustive list of responsibilities. Supervisory/Management Responsibilities This is a non-management job that will report to a supervisor, manager, director or executive. Minimum Requirements Education - Associate degree in Nursing. Bachelor's degree in Nursing preferred. Experience - Three (3) years of nursing experience. In Lieu Of NA Required Certifications, Registrations, Licenses Licensed to practice as a Registered Nurse in South Carolina.
