Develop and update a care plan in collaboration with physicians, the patient, and caregivers. Educate patients and caregivers on treatment, medication, and health management. Ensure interdisciplinary collaboration for holistic care delivery. Minimum Qualifications: New York State RN license Minimum of one year of recent hands on experience as a Registered Nurse. Basic Life Support Certification Valid New York State Driver’s License and reliable transportation Posted Salary Range USD $100,000.00 - USD $130,000.00 /Yr.
