Job Description: Registered Nurse (RN) Case Manager, Home Health Rhode Island Southern Territory | Sign-On Bonus Up to $7,500 Bring your nursing experience to a role where you can spend meaningful time with each patient. Elara Caring is seeking a full-time Registered Nurse to provide home health care throughout Rhode Island, including Westerly, North Kingstown, South Kingstown, Newport, Jamestown, and surrounding communities. You will manage your patient visits with independence while receiving support from an experienced clinical team. You will build trusted relationships, coordinate individualized care, and help patients remain safe and independent in the place they know best: home. Sign-on bonus of up to $7,500 available for eligible candidates Meaningful, one-on-one patient care Autonomy in the field Flexible scheduling Supportive and collaborative clinical team Competitive compensation Medical, dental, and vision insurance 401(k) with employer match Paid time off and paid holidays Tuition reimbursement for full-time employees Continuing education opportunities Career development and advancement opportunities Family and pet bereavement benefits Pet insurance What You’ll Do Manage an assigned caseload of home health patients Complete initial nursing evaluations and ongoing reassessments Develop, implement, and update individualized plans of care Assess patients’ physical, psychological, social, environmental, and caregiver needs Provide skilled nursing care in patients’ homes Educate patients and caregivers about medications, treatments, disease management, and safety Monitor patient progress and adjust care based on changing needs Communicate changes in condition to physicians, clinical leadership, and other members of the care team Coordinate services with nurses, therapists, physicians, caregivers, and community resources Complete accurate and timely clinical documentation Protect patient information and maintain compliance with HIPAA requirements Help patients achieve the highest possible level of health, safety, and independence What You’ll Bring Current and unrestricted Rhode Island RN license Bachelor’s degree in nursing At least one year of nursing experience in a clinical care setting Strong assessment, communication, organization, and patient-care skills Ability to manage an assigned caseload and work independently in the field Previous home health or hospice experience is preferred Previous Homecare Homebase experience is preferred Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
