The Job/What You’ll Do:
The Provider Support Services develop relationships with external Housing and Health Opportunities (H2O) providers to build and support the H2O network.
Responsibilities:
Serve as the primary point of contact and subject matter expert on the H2O program for contracted providers
Manage a high volume of provider inquiries that require research, input, and follow-up to ensure accurate resolution and communication
Documents and maintains records relative to communications with assigned providers to ensure a mutual understanding of expectations, deliverables, and necessary follow-up
Plans and leads regular on-site and/or virtual meetings, educational seminars, etc. with contracted providers
Educates H2O providers regarding appropriate H2O program requirements, including best practices in homelessness services delivery
Collaborates with various departments or designees to develop training and education as required
Conducts ongoing review of provider performance and engages in program improvement conversations
Assists in care coordination for complex cases
Demonstrates ability to plan, organize, prioritize, and work independently and consistently with limited supervision
Must be comfortable working in the field with community partners while working independently and in an autonomous manner

Education & Experience:
3-5 years of experience in Health, Human or Social Services, or related field, Preferred
Bachelor's Degree in business, social work, or related field, Preferred
Schedule:
Standard Business Hours, Monday-Friday 8 a.m. to 5 p.m.
Work Location: Remote (Applicants must reside in Arizona)
The candidate's applicable education and experience will determine where a candidate falls in the position range.
Pay Range $46,000 — $53,600 USD Working Conditions:
While performing the job duties in the office or at a home office, the employee is frequently required to stand, walk, sit, and use hands; they must occasionally lift and/or move up to 25 pounds. Specific vision abilities required by the job include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus. Exposure to noise typical with office operations. Ability to hear and speak.
