In this role, you will evaluate and treat residents, develop individualized treatment plans, and collaborate with interdisciplinary team members to help residents regain mobility, improve function, and maximize independence. Key Responsibilities Evaluate residents and develop individualized treatment plans based on physician orders and clinical needs Provide skilled physical therapy services to improve mobility, strength, balance, endurance, and functional independence Monitor resident progress and modify treatment plans as appropriate Communicate regularly with physicians, nursing staff, residents, families, and interdisciplinary team members regarding treatment goals and outcomes Complete accurate and timely documentation in accordance with facility, state, federal, and payer requirements Participate in care plan meetings, rehabilitation conferences, and discharge planning activities Educate residents, families, and caregivers on therapy goals, safety techniques, and home exercise programs Conduct in-service education and training for facility staff as needed Ensure compliance with all state practice acts, facility policies, and regulatory requirements Promote resident safety, independence, and quality of life Supervisory Responsibilities Supervise Physical Therapist Assistants (PTAs), therapy aides, and students in accordance with state regulations and facility policies Qualifications Education & Experience Bachelor's degree in Physical Therapy required Master's or Doctorate degree in Physical Therapy preferred Experience in skilled nursing, rehabilitation, or long-term care preferred Experience with Casamba and PointClickCare (PCC) preferred Strong clinical, communication, and documentation skills Licenses & Certifications Current Physical Therapist license in South Carolina and in good standing Must maintain all required continuing education requirements Must remain in good standing with the applicable State Board What Makes You a Great Fit Passion for helping residents regain mobility and independence Strong clinical judgment and problem-solving abilities Excellent communication and interpersonal skills Compassionate and resident-focused Team-oriented and collaborative Committed to delivering exceptional therapy outcomes We are an equal opportunity employer.
