Modern therapy gym and rehabilitation equipment Supportive interdisciplinary team environment Strong clinical mentorship and leadership support Opportunities for professional growth and advancement Continuing education support Patient-centered care model focused on outcomes Stable organization with a strong reputation for quality care Make a meaningful impact in the lives of patients every day Full-Time Benefits Include Medical, Dental, and Vision Insurance 401(k) Retirement Plan Paid Time Off (PTO) Holiday Pay Continuing Education Assistance Career Growth Opportunities Employee Recognition Programs $5,000 Sign-On Bonus Physical Therapist (PT) Responsibilities Evaluate patients and complete comprehensive physical therapy assessments Develop individualized treatment plans based on physician referrals, patient goals, and clinical needs Provide evidence-based physical therapy interventions to improve mobility, strength, balance, endurance, and functional independence Monitor patient progress and modify treatment programs as appropriate Supervise Physical Therapist Assistants (PTAs) and rehabilitation support staff Collaborate with physicians, nurses, social services, and interdisciplinary healthcare teams Participate in care planning meetings, discharge planning, and family conferences Educate patients, families, and caregivers regarding treatment plans, exercise programs, and safety strategies Maintain accurate, timely documentation in accordance with regulatory and facility standards Recommend and assist with adaptive equipment and durable medical equipment needs Ensure compliance with Medicare, state, federal, and facility guidelines Qualifications Current California Physical Therapist (PT) License in good standing Graduate of an accredited Physical Therapy program Bachelor's Degree required; Master's or Doctorate in Physical Therapy preferred Experience in Skilled Nursing Facility (SNF), Post-Acute Care, Rehabilitation, Long-Term Care, or Acute Care settings preferred Strong patient assessment and treatment planning skills Excellent communication and documentation abilities Knowledge of EMR systems such as Casamba or PointClickCare preferred Ability to work effectively in a collaborative healthcare environment New graduates with strong clinical experience are encouraged to apply About Lakeport Post Acute At Lakeport Post Acute, we believe exceptional patient outcomes begin with exceptional clinicians.
