RESPONSIBILITIES AND DUTIES - Pharmacist | Clinical Pharmacy Services [QNBMHF0028007] Dispense medications and provide direct patient counseling on proper use, side effects, and interactions. Verify, interpret, and assess completeness and appropriateness of prescriptions. Utilize ScriptPro and Pyxis systems for prescription management. Compound and label IV additives; manage unit dose carts in inpatient care. Conduct clinical chart reviews, renal function assessments, and medication reconciliation. Counsel patients on discharge medications. Collaborate with medical staff to resolve medication-related issues. Maintain DEA documentation and records of controlled substances (including Schedule II drugs). Participate in quality improvement programs per JCAHO and HCFA standards. Current, unrestricted pharmacist license from any U.S. state. Must meet IHS qualification standards for Registered Pharmacists. Fluent in English (speaking, reading, writing, and understanding).
