Pharmacist – Adult Internal Medicine Focused – Overnight Rotation

Union Position: No Department Details This position includes an overnight rotation every sixth week. The overnight rotation runs Tuesday through Monday for seven consecutive 10-hour shifts, from 8:30 p.m. to 6:30 a.m. Following the overnight rotation, the pharmacist is off until the subsequent Monday. The remaining four weeks of the rotation consist primarily of 8-hour day shifts, with occasional evening shifts. Summary The overnight rotation offers an every 6th weekend schedule with 7 consecutive overnights followed by 6 days off and then 4 weeks of weekday daytime shifts. Overnight shift start time is 8:30 pm and daytime shift start time varies from 5 am-7:30 am. The Adult Internal Medicine Focus Pharmacist provides direct pharmaceutical care to adult inpatient populations on medical and surgical units. Decentralized to patient care areas, this pharmacist works closely with providers, nurses, and other interdisciplinary team members to optimize medication therapy and promote safe, effective, evidence-based care. Job Description Key responsibilities include prospective patient profile review, participation in interdisciplinary rounds, identification and resolution of medication-related problems, patient education, and clinical consultation for healthcare professionals. The pharmacist also performs medication order verification, evaluates medication therapy for accuracy and appropriateness, and provides guidance regarding dosing, drug interactions, therapeutic monitoring, adverse effects, and medication administration. This role supports operational and distributive pharmacy functions as needed while promoting medication safety, regulatory compliance, and continuity of care in a fast-paced healthcare environment. Qualifications In a medical center or institutional setting: Graduate of an accredited School of Pharmacy. Must have a thorough knowledge of drugs and their actions; knowledge of state and local regulations. Hospital experience or residency preferred. Graduate of an accredited School of Pharmacy. Completion of an ASHP-accredited PGY1 pharmacy residency or equivalent experience preferred. License must be in good standing by the State Board of Pharmacy in state(s) of practice and/or possess multi-state licensure privileges as required by position.
