Job Details Responsibilities: Collaborates and coordinates with multidisciplinary teams to facilitate a safe and quality experience for patients through the continuum of care. Liaison between all patient care services to ensure appropriate bed is assigned, patient care needs are met and treatment plans are executed in the appropriate level of the continuum Applies advanced clinical judgment and technical proficiency to all patient placement decisions Manage real-time patient volume and eliminate flow delays Assign patient beds based on medical necessity, acuity level, required nursing care hours, and infection control isolation protocols Ensure regulatory compliance with EMTALA, HIPAA, and Joint Commission standards during all patient placement activities Qualifications: Passion for Care Coordination 1+ years of Case Management experience as a Registered Nurse (RN) 1+ years of nurse leadership experience Education: Bachelors of Science in Nursing (BSN) degree Licensure: New York Registered Nurse (RN) License & Registration Certification: Basic Life Support (BLS) Knowledge of Outlook, Word, Excel Must be flexible and able to work in a stressful environment Posted Salary Range USD $65.20 - USD $78.24 /Hr. This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate’s qualifications, skills, competencies and experience. The salary range or rate listed does not include any bonuses/incentive, differential pay or other forms of compensation that may be applicable to this job and it does not include the value of benefits.
