Job Details  As a Manager of Care Coordination, you will discuss discharge needs with Physicians and Social Workers; and partner with unit nurses to communicate with patients/family about admission and plan of care.  Qualifications: Active NY Registered Nurse license 2-3 years experience in health care related discipline and/or discharge planning or community healthcare experience BSN preferred Excellent interpersonal skills Ability to respond to short deadlines Posted Salary Range USD $50.64 - USD $73.44 /Hr. This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate’s qualifications, skills, competencies and experience.
