Licensed Practical Nurse (LPN) - Hospice

We CARE for our patients like they are our own FAMILY.                 Schedule: Monday - Friday, 8:15AM - 5:00PM with on-call requirements Agency: Mercy Hospice SUMMARY   The Licensed Practical Nurse I (LPN), Hospice provides comprehensive skilled care as ordered by the attending physician/medical director by following the order sets created by the RNCM. The LPN I educates patients and their family members and ensures the safety of the patient. The LPN I is accountable for completing accurate documentation and remains compliant with all legal rules and regulations, as outlined by the state.   KEY RESPONSIBILITIES   Provides skilled nursing care in a patient’s home as ordered by the attending physician/medical director   Performs care by following order sets and goals made by RN   Observes and monitors patient conditions   Notifies RNCM of any changes in patient condition   Reconciles medications prescribed by the physician   Counsels patient and family on the disease/progression and how to manage   Incorporates patient and family in development of plan of care   Listens to patient and family members   Completes all clinical documentation in accordance with agency protocol and Medicare/Federal guidelines   Documents all aspects of treatment, assessments, and patient education   Communicates with scheduler any changes outside the normal   Participates in all on-call requirements and case conferences   Attends in-service trainings and mandatory agency meetings   Maintains a professional appearance as a representative of the company   Understands and practices agency policies and procedures   Other duties as assigned   QUALIFICATIONS   One year of experience as an LPN in an acute care setting is strongly recommended. Hospice care experience is desired.   Ability to create positive impressions and communicate with a variety of people. Maintain effective communication with patients, families, physicians and co-workers. Ability to remain calm, have patience and be accommodating. Compassionate and caring while working with patients and families.   Knowledgeable on nursing best practices. Ability to make appropriate nursing judgments. Ability to identify a situation and handle it with the best possible solution. Detail-oriented and observant.   Disciplined style of work ethic. Ability to prioritize and be timely .
