Location: Candidate must reside in California Remote with up to  75% regional travel during new center launches Estimated Start Date:    ASAP Role Scope : The Functional Wellness and Rehabilitation Educator will lead the onboarding and training of Habitat team members on-site at PACE Centers, with an emphasis on Physical Therapists (PTs), Occupational Therapists (OTs), Activities Coordinators, and Personal Care Attendants (PCAs). This role works as an extension of the Centralized Education Team within the Practice Operations team, leading and providing oversight of onboarding, practicum and shadowing activities of assigned roles at the Center. This role will also be an operational subject matter expert, helping ensure consistency in onboarding, training, workflows, documentation, and best practices. The Functional Wellness and Rehabilitation Educator will be responsible for delivering didactic trainings, conducting competency reviews, competency signoffs, skills validation, and preceptor checklist oversight for team members as they progress through their onboarding process. This role will partner closely with Practice Operations, Center Leadership, People Team and the other functional educators to maintain and optimize workflows, training materials, and operational resources ensuring clinical and operational best practices are implemented consistently across the organization. The position balances direct educational responsibilities with process improvement and program development activities to support high-quality participant care, operational excellence, regulatory compliance, and team member engagement.   Core Responsibilities & Expectations for the Role   Field based owner of training oversight and fidelity for assigned roles— PT, OT, Activities Coordinator and PCA’s, and others as needed.    Leads the on-site learning experience for existing and new center launches within their assigned roles across centers.    Manages the training, practicum, and shadowing assignments and logistics within the Centers for new team members.   Collaborates with Center leadership to oversee the completion of competency signoffs, skill validation, and preceptor checklists for the necessary roles in the Center.   Performs scheduled visits to all assigned centers in the region supporting leaders and Center Champions with ongoing learning needs.   Partners with L&D central hub to design and coordinate on site learning opportunities that support role readiness and capability development.   Works with SMEs, Center Leaders, and People Team partners to ensure learning programs meet business needs and reinforce Habitat’s culture and capabilities.   Serve as a subject matter expert for assigned roles operations, processes, and best practices.   Collaborate with leaders and stakeholders to develop, document, and improve workflows and standard operating procedures.   Review existing processes and recommend improvements to increase efficiency, quality, compliance, and consistency.   Create, update, and maintain role playbooks, reference materials, job aids, SOP’s, and educational resources.   Required Qualifications:    Active California PT or OT license.   3–5+ years of experience teaching or training in the field.   Demonstrated experience in facilitation and communication skills.   Experience delivering learning programs.   Experience developing workflows, SOPs, training materials, or process improvements.   Ability to coach others, including leaders, using structured frameworks.   Strong relationship building and stakeholder management skills.   Aligns with our purpose and our values, and is excited about living those out in daily practice   Ability to thrive in a fast-paced, evolving environment with comfort navigating ambiguity, adapting quickly, and contributing to continuous improvement   Strong learning and growth mindset, including seeking feedback, engaging in healthy debate and using data and curiosity to inform decisions.   Acts with integrity and ownership, considering the broader organizational impact, doing the right thing, and following through to deliver results.   Nice to have:   Experience working in a value-based care or elder care/PACE environment.   Essential Functions & Physical Requirements   The following statements describe the general nature of work and physical expectations common across roles at Habitat Health.
