Posting Date 09/10/2026 2000 16th St, Denver, Colorado, 80202-5117, United States of America About the Role DaVita Integrated Kidney Care (IKC) is seeking a Director, Clinical Programs to lead high-priority care-model transformation and clinical-performance initiatives across our value-based care portfolio. This role is for a strategic healthcare operator who knows how to turn clinical and business strategy into something that works in practice. You will lead complex initiatives from design through implementation, stabilization, and ongoing performance improvement, bringing together clinical, operational, analytical, product, technology, and financial teams around shared outcomes. The initial portfolio will include care-model transformation for patients with chronic kidney disease and broader initiatives focused on improving clinical outcomes, reducing avoidable utilization, advancing quality, and improving affordability. The portfolio will evolve as enterprise priorities change. DaVita is a global leader in kidney care, and IKC is helping extend that platform across a broader continuum of care. As commercial payers and CMS continue to use value-based and risk-based arrangements in kidney care, this role offers the opportunity to help shape how those models work at scale - from strategy to day-to-day care delivery and measurable performance. This is a high-ownership role with meaningful exposure to senior leaders and the opportunity to shape how care is delivered across a growing value-based care business. What You Will Do LEAD CARE-MODEL TRANSFORMATION • Lead the design, implementation, and improvement of care models serving patients with complex chronic conditions. • Translate clinical and business priorities into practical operating models, workflows, roles, accountabilities, and management routines. • Identify the most important barriers to performance and bring the right teams together to solve them. • Build approaches that can scale across programs, populations, and markets. DRIVE CLINICAL AND OPERATIONAL PERFORMANCE • Lead initiatives designed to improve patient engagement, clinical outcomes, quality, avoidable utilization, and total cost of care. • Partner with clinical and operational leaders to translate patient-risk and performance insights into focused interventions. • Evaluate whether operational activity is producing the intended patient and business outcomes. • Use data and frontline insight to identify performance gaps and focus teams on the actions that matter most. LEAD FROM STRATEGY THROUGH EXECUTION • Own complex initiatives from planning and readiness through implementation, stabilization, and sustainable operations. • Establish clear priorities, accountabilities, risks, and decision points. • Ensure teams have the workflows, tools, training, reporting, and support needed to execute successfully. • Resolve cross-functional issues through durable solutions rather than recurring workarounds. BUILD EFFECTIVE PERFORMANCE SYSTEMS • Partner with Analytics, Product, and Technology teams to develop reporting and tools that enable leaders to manage performance. • Connect strategic goals with operational measures, clinical outcomes, and financial performance. • Help leaders identify where execution is breaking down and take timely action. • Translate complex performance information into clear insights and recommendations. LEAD ACROSS THE ENTERPRISE • Lead highly matrixed work across Clinical, Operations, Analytics, Product, Technology, Patient Engagement, Finance, Actuarial, and other functions. • Build trusted relationships with senior leaders and cross-functional partners. • Facilitate decisions when teams have different priorities, perspectives, or constraints. • Communicate complex issues simply, including clear recommendations, tradeoffs, and decisions required. DEVELOP PEOPLE AND CAPABILITY • Lead and develop managers, analysts, and workstream leaders. • Create meaningful ownership and clear accountability within the team. • Coach others in structured problem-solving, stakeholder leadership, operating judgment, and executive communication. • Build repeatable approaches that strengthen the organization's transformation capability.
